ACE was founded in May 2020. We provide Smart and/or Hot Spot device(s) to undeserved populations with a need to connect to care for virtual (telehealth) doctor appointments. A Smart device is required for virtual appointments and a WiFi connection is required to use the Smart device. ACE partners with physician offices, health centers, mental health clinics, churches, schools, group & foster homes, and shelters to identify those in need of Smart and/or Hot Spot device(s). ACE's priority populations are patients diagnosed with chronic illnesses such as HIV/AIDS, Cancer, Alzheimer's, Arthritis, Diabetes, Mental Health Disorders, Heart Disease, and Disabilities. As proven during the COVID-19 global pandemic, telehealth improves health outcomes by bridging the digital divide and creating access and equity to healthcare; for in many instances, telehealth was the only healthcare delivery model available.
ACE is committed to helping eliminate the transmission of HIV and believes health education is invaluable to this effort. Therefore, ACE partners with minority churches to create a safe space for congregants to have support, become informed about treatments, debunk stigma, learn of resources, and receive encouragement of the good quality of life possible beyond the HIV diagnosis. ACE links participants to primary care providers (PCPs) through our Connection 2 Care program. We believe in an integrated and value-based approach to care, as this builds a healthcare team around the patient, encouraging continuity of care. WHATWhat is ACE doing? WHYWhy ACE? Here at ACE, we are passionate about health equity and believe everyone deserves access to high-quality healthcare; therefore, we want to help close telecommunication gaps for low-income patients needing access to healthcare.
WHOWho does ACE serve? ACE is excited to serve underrepresented, underserved, marginalized, and low-income populations. Priority populations are participants receiving one or more of the following: SNAP, TANF, Medicare, Medicaid, SSI, SSDI, disabled veteran, unemployed, and/or have an annual household income of $25,000 or less. WHEREWhere is ACE's service area? Although headquartered in Atlanta, Georgia - ACE serves any qualifying participant within the United States.
HOWHow is ACE funded? ACE operates with funds from grants, individual donors, donations, foundations, fundraisers, charity events, partnerships, and sponsorships.
